This Privacy Collection Notice explains how we collect, use, and protect your data, ensuring it is handled with care and in compliance with privacy laws.
Your Personal Information and your patient Personal Information is being collected by the National Heart Foundation of Australia ABN 98 008 419 761 (Heart Foundation, we, us, our) to support participation in our Walking Program (the Program). This is to enable us to effectively action your patient referral to our Program and for other purposes as set out in our Privacy Policy. The patient information will be used to make initial contact to action your referral and only retained with their ongoing consent to participate in the program. We will also retain your contact information to keep you informed on relevant Health professional programs and information.
Providing us with the requested personal information is not required by law, but if you do not provide us with this information, we may be unable to action your referral request. Communications from us may include mail, email, through social media (including targeted online advertising), SMS or telephone contact. If you do not want to receive direct marketing communication from us, you can unsubscribe
using the link in the communication or otherwise contact us using the details below.
